Filling out the Liability Waiver also creates your profile with APEX. The password you create will be the password you use to access your account with us through the MindBody Online portal.

If you are filling the form out for yourself and your minor child(ren), click  it may be necessary for us to reset your password in order for you to fully access your accounts.

The top part is the information of the parent or participant over 18 years of age. You then click ADD A CHILD and enter information for children.*

*A legal guardian must complete the Liability Waiver for minors. A person misrepresenting themselves as legal guardian for a minor accepts sole responsibility and liability for that minor.
